This isn't my cup of tea for pleasure (although I do have a busy dungeon in my mind), so I can't provide any practical advice about how to do extreme routines. I can tell you that an infection (at least cellulitis and maybe something worse) is not very exciting, but the likelihood of an infection can be minimized. If you take care of details ahead of time, prep shouldn't get in the way of a good scene.
"medical alcohol" (like what - isopropyl or rubbing alcohol?) does kill bacteria and viruses, but generally not on contact. Time is required. So, objects which are going to pierce the skin and come into contact with the bloodstream should be soaked in rubbing alcohol (70% - 90%) for at least 15 minutes. If they can be immersed in boiling water, even better - put the objects in the boiling water, boil for at least a minute or two, then cover and turn the heat off. Leave them in the covered water until they are cool, or until you are ready to use them.
Rubber gloves that you get from the drugstore are a barrier between your blood and somebody else's. The gloves are not themselves sterile. Ordinary rubber gloves can not be heat treated. They can be washed, once they are on the hands.
The skin of the penis, scrotum, crotch, nipple, or ... wherever, needs to be prepped before being worked on. Soap and water is OK, IF nothing else is available. Scrub, but don't scrub so hard that the skin is scratched and scuffed, which just makes infection more likely.
If you have the time to get ready, get a bottle of Hibiclens (an over-the-counter product used as an at-home surgical prep) at your drugstore. It'll cost you a few dollars. It is an effective antiseptic/disinfectant. (Note: Hibiclens does stain, so use it in the shower and rinse off thoroughly.) No product will sterilize the skin, but you want to reduce the amount of bacteria and fungus on the relevant skin to a minimum before the games begin. Read the instructions.
The guy who is going to do the extreme routines on you needs to have hands as clean as your skin, or it won't make much difference that your skin was prepped.. He should wash his hands with Hibiclens. The non-rubber gloves should be able to take Hibiclens once they are on the hand. Try it ahead of time.
The surface on which all of the planned activities are going to be done should be pretty clean before you start. Hard surfaces can usually be scrubbed with soap and water and maybe have some chlorine bleach applied to them (it doesn't have to be straight from the bottle - 1 part bleach to 10 parts water will work. The bleach solution doesn't kill bacteria on contact either, so give it a few minutes to work before wiping it off.)
Softer surfaces which can't be cleaned too much (leather, for instance) should be covered with cloth that has been freshly laundered in hot water and dried in a hot drier.
Sounds complicated, but it really isn't that bad, and I'm assuming that this will get done before the guys arrive.
One more thing: Certainly there are a lot of trust worthy people out there. Just be sure that you make the judgement about their reliability based on some sort of evidence (like you know they are actually experienced and are capable of being careful). Good looks and sex appeal do not guarantee competence. For god's sake, start small - you don't have to dive into the deep end of the pool to learn how to swim.
